WhatsApp just changed how it bills clinics. Here's what it actually costs you.

Short answer: from 1 October 2026, WhatsApp charges for replies to patients after the first 1,000 a month, and for confirmations sent while a patient is chatting. For a clinic with 400 appointments a month, that adds roughly ₹326 a month including GST. The bigger costs are chatbots that send too many messages and provider markups you can't see.

From 1 October 2026, WhatsApp starts charging for two kinds of messages that used to be free. If your clinic uses the WhatsApp Business API for reminders, bookings or a chatbot, your bill changes this month.

The good news: for most clinics the extra cost is small. The bad news: nobody tells you how small, so a lot of clinics will either panic or get overcharged. Here is the plain version.

What changed on 1 October

WhatsApp (Meta) charges per message delivered. Until now, two things were free:

  1. Replies inside the 24-hour window. When a patient messages you, you get 24 hours to reply. Those replies, from your staff or a chatbot, were free.
  2. Utility messages sent inside that window. A booking confirmation sent while the patient is still chatting was also free.

From 1 October, both are charged. Each WhatsApp number gets 1,000 free service replies a month. After that, every reply is charged at the utility rate. Utility messages sent inside the window get no free allowance.

The current rates for India

Message typeWhat it's for in a clinicMeta's rate per message
MarketingHealth camp offers, festival greetings, "come back for a check-up" campaigns₹0.8631
UtilityAppointment confirmations, reminders, report-ready alerts₹0.115
AuthenticationOTPs for patient login₹0.115
Service (new)Replies to patient messages, after the first 1,000 a month₹0.115

Add 18% GST on top. Your WhatsApp provider (the company that set up your API) also adds its own platform fee or per-message markup.

What a typical clinic will actually pay

Illustrative example, not real clinic data: a clinic with 400 appointments a month.

New cost: about ₹276 a month, or ₹326 with GST. Less than one missed appointment.

So the Meta price change is not the problem. Three other things are.

Where clinics actually lose money

1. Chatty bots. A chatbot that sends three short messages where one would do triples your reply count. A bot that answers "ok" with "Thank you! Is there anything else I can help you with?" pays for that line every time.

2. Hidden markups. Meta's rate is the same with every provider. What differs is what your provider adds on top. Some add ₹0.05–₹0.30 per message, which can be more than Meta's own utility rate. Ask for Meta charges and platform fees as separate lines on your invoice.

3. Marketing sent as blasts. At ₹0.86 per message, a festival greeting to 3,000 old patients costs about ₹2,600 plus GST. Worth it if it brings patients back. Wasteful if half the list hasn't visited in three years.

4 ways to keep the bill small

  1. Write one clear message instead of three. Put the date, time, doctor, address and a reschedule option in a single reminder.
  2. Let the chatbot stop. When the patient's question is answered, the conversation should end. No filler replies.
  3. Send marketing only to recent, consenting patients. Patients seen in the last 12–18 months who agreed to hear from you. It is cheaper, and under India's new data protection rules it is also safer. See Is your clinic's WhatsApp ready for India's new data protection law?
  4. Check your invoice monthly. Count the messages by type. If service replies jump, find out which flow is sending them.
